Panic attack or heart attack? How to think safely

Panic can feel cardiac. Safety comes first; then treatment prevents the loop from taking over.


Chest tightness, racing heart, sweating and breathlessness should be taken seriously. The first episode, or any unusual severe episode, deserves medical evaluation. After clearance, the work becomes preventing fear of the sensation from becoming the disorder.

What I see clinically

Many panic patients have repeated emergency visits because the body sensations are terrifying. Each clean report reassures briefly, then the next sensation restarts doubt. The brain learns that the heart must be checked every time anxiety spikes.

What to do this week

If symptoms are new, severe, exertional, associated with fainting, radiating pain, known cardiac risk or uncertainty, seek emergency care. If a doctor has cleared you and the pattern is recurrent panic, build a written panic protocol: name it, slow exhale, grounding, movement after the wave, and follow-up therapy.

When to get help

Do not self-diagnose chest pain from an article. But do not let repeated cleared panic attacks remain untreated either. Panic disorder is highly treatable with structured therapy and, when needed, medication.
